1. How Can I Enroll My company With Vision Select?

Call toll free 1-800-641-3809 and a Vision Select representative will
assist you in the enrollment process.

2. What is the Enrollment Process?

Once you’ve spoken with a Vision Select representative by phone, you will be sent
a packet containing presentation literature, brochures, an introductory letter,
and request form. Mail or fax your completed and signed Request Form back to
Vision Select and a membership number will be generated for you.

3-A. For Members:

You can register yourself and your family by entering your five-digit eligibility number and completing the Membership Registration Form. Then click on “Print your card”.

3-B. For Brokers:

Brokers can register
clients by entering
their five-digit eligibility number and completing the Broker-Client Enrollment Form. Then click on “Print your card”.

4. Who are the Vision Care Providers?

Vision Select is able to offer incredible eye-care discounts by our participatory eye care providers and The LASIK Vision Institute centers nationwide. Please see our Provider List to find a location nearest you. Be sure to bring your membership card with you at the time of purchase.
